- Have your paperwork ready when you come to register:
- Proof of your child's age (birth certificate, passport, or record of baptism)
- Immunization records
- Two (2) of the documents below verifying proof of address:
- Lease agreement, deed, mortgage statement for the residence, residential utility bill (gas or electric) in the residents name issued by the utility company (such as National Grid or Con Ed).
- Have your paperwork ready when you come to register:
